The Alere Heart Failure Program  is an at-home program for patients with advanced heart failure. It provides a daily connection to patients that reinforces physician prescribed treatment plans. By placing Alere’s patented DayLink® monitor in patients' homes to track weight and heart failure symptoms, the Alere Heart Failure Program facilitates efficiency for the physician by providing timely, actionable information. It gives physicians real-time biometric and symptomatic information when early clinical intervention is needed to improve their patient's health and prevent costly hospitalizations. The program is supported by the American College of Cardiology and American Heart Association ACC/AHA Diagnosis and Management of Chronic Heart Failure in the Adult 2005 (http://www.americanheart.org/).
 
The Alere Heart Smart Solutions™ Program for Coronary Artery Disease provides a comprehensive plan for patients with Coronary Artery Disease (CAD). Clinical interventions are supported by evidence-based guidelines and promote a collaborative relationship between the physician and patient. Combining medication monitoring, motivational interviewing techniques, and behavioral science, the Alere CAD Program generates reduction of trends in myocardial infarctions, mortality rates and CAD-related admissions while improving patient’s quality of life. The program is supported by the American College of Cardiology and American Heart Association (ACC/AHA) evidence-based guidelines for Cardiovascular Disease Prevention in Women 2004; ACC/AHA Guideline for the management of Patients with Chronic Stable Angina 2002; ACC/AHA Clinical Performance Measures for Chronic Stable Coronary Artery Disease 2003 (http://www.americanheart.org/); Institute for Clinical Systems Improvement (ICSI) Stable Coronary Artery Disease 2006, (http://www.icsi.org/).
 
The Alere Asthma Program powered by National Jewish provides a comprehensive plan for patients with Asthma. Alere Medical has partnered with the National Jewish Research and Medical Center (the #1 Respiratory Hospital in the U.S.) to develop this program. Clinical interventions are supported by the Guidelines for the diagnosis and management of Asthma, NHLBI, National Asthma Education and Prevention Program, and National Jewish Medical and Research Center. The Alere-National Jewish Respiratory Care Program reduces medical service utilization (ER visits, unscheduled Doctor Visits, etc.) as well as missed days for work and school, and improves patients’ quality of life. The program is supported by the 2002 Diagnosis and Management of Asthma practice guidelines (http://www.nhlbi.nih.gov/).
 
The Alere COPD Program powered by National Jewish provides a comprehensive plan for patients with Asthma or Chronic Obstructive Pulmonary Disease (COPD). Alere Medical has partnered with the National Jewish Research and Medical Center (the #1 Respiratory Hospital in the U.S.) to develop this program. Clinical interventions are supported by the GOLD and GOLD II evidence-based guidelines (Global Initiative on Obstructive Lung Disease) from WHO, NHLBI and National Jewish Medical and Research Center. The Alere-National Jewish Respiratory Care Program reduces medical service utilization (ER visits, unscheduled Doctor Visits, etc.) as well as missed days for work and school, and improves patients’ quality of life. The program is supported by the 2006 Global Strategy for the Diagnosis, Management and Prevention of Chronic Obstructive Pulmonary Disease practice guidelines (http://www.goldcopd.com/).
 
The Alere Diabetes Program powered by Joslin Diabetes Center is a comprehensive program for patients with diabetes, developed in collaboration with Joslin Diabetes Center, the global leader in diabetes research, care and education. Clinical interventions are supported by Joslin’s clinical practice guidelines and promote a collaborative relationship between the physician and patient. Combining biometric monitoring, motivational interviewing techniques, medication review and technology, the Alere Diabetes Program powered by Joslin results in improved key health numbers, increased compliance with treatment plans and preventive exams, and reduction in diabetes-related utilization. Unique interventions are supported by Joslin Clinical Guidelines for Adults with Diabetes 2006 (http://www.joslin.org/) and the American Diabetes Association’s Standards of Medical Care in Diabetes 2006 (http://www.diabetes.org/).
